ACCESS, defined
Advancing Chronic Care with Effective, Scalable Solutions is a CMS Innovation Center model in Original Medicare. It pays participating providers for technology-supported chronic care with payment tied to outcomes. HealthyConnect serves as the specialized ACCESS provider; your organization does not need to become an ACCESS participant to refer patients or perform eligible co-management.
