- What changed
- The patient reports difficulty following the medication schedule. Recent home blood-pressure readings and the available medication list are assembled for review.
- What HealthyConnect did
- The care team clarified the patient’s questions, gathered the available readings and medication information, and routed the concern to the designated clinician.
- What needs your review
- Review the medication information and the patient’s reported difficulty. Consider whether reconciliation or updated follow-up instructions are appropriate.
- What happens next
- The reviewing clinician documents the clinical decision and communicates follow-up instructions. HealthyConnect follows up with the patient and incorporates the agreed next steps into ongoing support.
