What changed with ACCESS?

ACCESS is a Medicare payment model for ongoing, technology-supported chronic care. HealthyConnect delivers that care alongside a patient’s existing providers.

What does ACCESS stand for?

Advancing Chronic Care with Effective, Scalable Solutions. CMS administers the national model, which began July 5, 2026, for Original Medicare. Participating care organizations receive recurring payments linked to health outcomes.

Why does it change care between visits?

HealthyConnect takes responsibility for the ongoing ACCESS care operation: patient support, progress tracking, care updates and its own billing. Practices connect patients with a specialized care partner. They do not have to build an ACCESS delivery organization to work with HealthyConnect.

Which conditions does ACCESS address?

Four tracks are available in the model now, with four additional condition tracks announced for April 1, 2027. See who can benefit and explore all eight tracks.

How is it different from CCM, RPM and RTM?

ACCESS links payment to outcomes. CCM, RPM and RTM have their own service and billing requirements. For partners, HealthyConnect provides a separate clinical care relationship and a coordinated flow of patient information.

What about Medicare Advantage?

The CMS model is for Original Medicare. ACCESS-aligned Medicare Advantage arrangements depend on the health plan; payer pledges describe potential expansion, not current coverage for every member.
