HEALTHYCONNECT · ACCESS EXPLAINED

Who is ACCESS for?

For patients whose chronic conditions need attention between appointments. ACCESS connects eligible patients with ongoing care alongside their existing clinicians. Start with the condition groups below; HealthyConnect confirms coverage, clinical eligibility and the services available to the patient.

Start with three questions.

Coverage

The CMS model is in Original Medicare. Medicare Advantage arrangements are plan-specific; a payer pledge does not confirm an individual benefit.

Condition

Does the patient meet an available track’s clinical criteria? A diagnosis or symptom alone does not complete the eligibility check.

Care connection

HealthyConnect checks available services and existing care arrangements, obtains consent and connects the patient with the appropriate pathway.

A track groups related conditions into one care framework. These examples explain the care opportunity; they are not eligibility determinations or promised outcomes.

01 · CURRENT MODEL TRACK

Early metabolic and blood-pressure risk

eCKM · Early cardio-kidney-metabolic

High blood pressure, abnormal cholesterol, prediabetes, or obesity / overweight meeting the model’s central-obesity criteria.

A patient you may recognize

A patient with high blood pressure who needs help putting the care plan into practice between visits.

What ongoing support can address

Follow relevant readings, support agreed lifestyle and medication plans, and bring changes back to the treating clinician.

02 · CURRENT MODEL TRACK

Diabetes, kidney and cardiovascular disease

CKM · Cardio-kidney-metabolic

Diabetes, stage 3a or 3b chronic kidney disease, or atherosclerotic cardiovascular disease.

A patient you may recognize

A patient with diabetes who needs ongoing support following the plan and tracking progress.

What ongoing support can address

Connect condition-related information, patient questions and progress so the care team can coordinate next steps.

03 · CURRENT MODEL TRACK

Persistent muscle, joint or back pain

MSK · Musculoskeletal

Chronic musculoskeletal pain meeting track criteria.

A patient you may recognize

A patient whose persistent pain makes daily activity difficult and who needs support between clinical encounters.

What ongoing support can address

Support the agreed activity and care plan, follow reported pain and function, and share barriers with the treating team.

04 · CURRENT MODEL TRACK

Depression and anxiety

BH · Behavioral health

Depression or anxiety meeting track criteria.

A patient you may recognize

A patient who needs help staying engaged with an agreed behavioral-health care plan between visits.

What ongoing support can address

Follow reported symptoms and engagement, identify barriers, and coordinate with the patient’s existing clinicians.

More than one condition? Related conditions within one track are managed together. A patient may qualify for more than one distinct track; eCKM and CKM are alternatives. Understand multiple tracks →

Heart failure

A separate track focused on people with heart failure.

COPD

A track for chronic obstructive pulmonary disease.

Substance use disorder

A track addressing substance use disorders.

Tobacco cessation

A track supporting people working to stop tobacco use.

CMS also announced a follow-on period for the existing musculoskeletal track. This is separate from the four additional condition tracks.

1. Connect

Patient directly: start at MyHealthyConnect.

Through a provider: your practice, specialist, therapist or pharmacy connects you with HealthyConnect.

2. Confirm

HealthyConnect checks coverage, qualifying conditions and service availability, and obtains consent.

3. Begin care

HealthyConnect establishes the care plan and connects ongoing support with the existing care team.

Is a physician referral required?

No outside physician referral is required for a patient to approach an ACCESS organization. HealthyConnect still completes the eligibility and clinical enrollment steps.

Does the patient keep their doctor?

Yes. HealthyConnect’s care connects with the patient’s existing providers. See how the care team works together.

What does the patient pay?

HealthyConnect’s stated offering waives patient cost-sharing for HealthyConnect ACCESS services. Other services the patient receives retain their own coverage and payment rules. See the CMS waiver and co-management sources.
