Who operates ACCESS?
CMS administers the national ACCESS model. HealthyConnect delivers ongoing patient care—handling enrollment, patient support, progress tracking and its own Medicare billing. Your practice keeps its patient relationships and can receive payment for qualifying clinical reviews.
You can partner with HealthyConnect without becoming an ACCESS participant.
Connect appropriate patients with HealthyConnect. HealthyConnect takes responsibility for its ACCESS care operation, while your team continues treating patients and coordinates through useful care updates.

What does this mean for your organization?
You have a specialized care partner for ongoing ACCESS services. Agree how patients connect, where HealthyConnect sends updates and who handles clinical follow-up. HealthyConnect manages its care delivery and bills Medicare directly for its ACCESS services.
Eligible clinicians can separately bill documented, qualifying co-management work. Participation in a patient’s care team does not automatically make every professional eligible for that payment.

HealthyConnect is part of the Remote Care Partners organization. MyHealthyConnect is the patient-facing connection to HealthyConnect’s care. These names connect the care organization and its technology; they do not refer to CMS itself.
